Dosage and direction
The recommended dosage is the inhalation of the contents of one Tiova Rotacap (18 mcg) once daily, at the same time each day, using the Rotahaler device. It is not to be taken more than once in a 24-hour period. The capsule must not be swallowed. Detailed steps for use are:
- Pull off the ribbed end of the Rotahaler transparent base.
- Place the Tiova Rotacap capsule upright in the square hole of the chamber.
- Push the transparent base back onto the mouthpiece until a click is heard.
- Hold the Rotahaler upright, exhale fully away from the mouthpiece, then place the mouthpiece in the mouth and close lips around it.
- Inhale deeply and steadily to pull the medication into your lungs.
- Hold your breath for up to 10 seconds, then exhale slowly.
- Open the Rotahaler to check if any powder remains; if so, repeat inhalation.
- Dispose of the empty capsule. Do not leave it in the chamber.
Precautions
- Inform your physician if you have a history of kidney problems, narrow-angle glaucoma, prostatic hyperplasia, or bladder neck obstruction.
- It may cause blurred vision or dizziness; exercise caution when driving or operating machinery.
- Immediate hypersensitivity reactions, including angioedema, may occur. Discontinue use and seek medical attention if such reactions develop.
- As an anticholinergic agent, it may worsen symptoms of urinary retention.
- Do not allow the powder to come into contact with your eyes, as it may cause pupil dilation and blurred vision or worsen narrow-angle glaucoma.
Contraindications
Tiova Rotacap is contraindicated in patients with a known hypersensitivity to tiotropium bromide, atropine or its derivatives, including ipratropium, or any component of the formulation.

Drug interaction
- Concomitant use with other anticholinergic-containing drugs (e.g., ipratropium, aclidinium) may potentiate anticholinergic adverse effects and is not recommended.
- While no specific pharmacokinetic drug interactions are known, always inform your doctor of all other medications you are taking, including other inhaled medications, over-the-counter drugs, and herbal supplements.
Missed dose
If a dose is missed, it should be taken as soon as remembered on the same day. However, if it is almost time for the next scheduled dose, the missed dose should be skipped and the regular dosing schedule resumed. Do not take a double dose to make up for a missed one.
Overdose
An overdose is likely to manifest as excessive anticholinergic effects, such as severe dry mouth, visual disturbances, tachycardia, and difficulty in micturition. In case of suspected overdose, symptomatic and supportive therapy should be instituted. Medical attention should be sought immediately.
Storage
- Store at room temperature (15° - 30° C), protected from light and moisture.
- Keep the capsules in the blister pack until immediately before use.
- Keep out of reach of children and pets.
- Do not store the capsule inside the Rotahaler device.
- Do not puncture or break the capsule.
