Common use

Tylenol is commonly used for the temporary relief of minor aches and pains. This includes headaches, including tension-type headaches; muscular aches; minor arthritis pain; toothaches; backaches; and the pain associated with the common cold or flu. It is also widely used to reduce fever. It is not indicated for the reduction of inflammation, as it lacks significant anti-inflammatory properties. It is appropriate for use in adults and children, though specific pediatric formulations and dosing must be followed meticulously.

Dosage and direction

Adults and children 12 years and older:

  • Take 2 caplets (1000 mg) every 4 to 6 hours while symptoms persist.
  • Do not exceed 6 caplets (3000 mg) in 24 hours, unless directed by a physician. Some experts recommend a maximum of 3250 mg in 24 hours, but a more conservative limit is often advised.

Children (Dosing based on weight is preferred over age):

  • Consult a doctor for children under 2 years of age.
  • For children 2 to 11 years: Use children’s or junior strength formulations and follow package dosing charts based on weight and age precisely.
  • The typical dose is 10-15 mg per kilogram of body weight every 4-6 hours, not to exceed 5 doses (75 mg/kg) in 24 hours.

Direction:

  • Take with a full glass of water.
  • May be taken with or without food.
  • Do not crush, chew, or dissolve extended-release tablets; swallow them whole.
  • For liquid formulations, use the measuring device provided to ensure accurate dosing. Do not use a household spoon.

Precautions

  • Do not use with other products containing acetaminophen. Acetaminophen is present in over 600 medications, including many prescription pain relievers and cold/flu formulations. Concurrent use can lead to overdose.
  • Consult a doctor if pain persists for more than 10 days or fever for more than 3 days.
  • If you are pregnant or breastfeeding, ask a health professional before use.
  • If you consume 3 or more alcoholic drinks every day, ask your doctor if you should take acetaminophen, as alcohol use can increase the risk of liver damage.
  • Patients with liver disease or a history of liver problems must consult a physician before use.
  • Do not use for marked redness or swelling, as these may indicate an inflammatory condition requiring a different therapeutic approach.

Contraindications

Tylenol is contraindicated in patients with:

  • Known severe hypersensitivity to acetaminophen or any of the inactive ingredients in the product formulation.
  • Severe hepatic impairment or active liver disease.
  • A history of allergic reactions (e.g., skin reddening, blisters, rash) to acetaminophen.

Possible side effect

Although generally well-tolerated, side effects can occur. In rare cases, acetaminophen can cause serious side effects. Stop use and seek medical help if you experience:

  • Allergic reactions: Skin rash, hives, itching, swelling of the face, throat, tongue, or lips, difficulty breathing.
  • Liver toxicity signs: Dark urine, clay-colored stools, nausea, vomiting, loss of appetite, pain in the upper right abdomen, yellowing of the skin or eyes (jaundice). These are signs of potential hepatotoxicity and constitute a medical emergency.
  • Less serious side effects are uncommon but may include headache or insomnia.

Drug interaction

Acetaminophen can interact with several medications, potentially altering their effects or increasing the risk of adverse events.

  • Warfarin (Coumadin): Long-term use of acetaminophen may potentiate the anticoagulant effect of warfarin, increasing the risk of bleeding. Monitoring of INR is recommended.
  • Isoniazid: This anti-tuberculosis medication may increase the risk of acetaminophen-induced hepatotoxicity.
  • Other Acetaminophen-containing products: As stated in Precautions, this is the most critical interaction risk.
  • Barbiturates, Carbamazepine, Phenytoin: These drugs may increase the metabolism of acetaminophen, potentially reducing its efficacy or increasing the production of toxic metabolites.
  • Alcohol: Chronic excessive alcohol consumption induces liver enzymes and depletes glutathione, significantly increasing the risk of severe liver damage from acetaminophen.

Missed dose

This medication is typically taken on an as-needed basis for pain or fever. It is not intended for scheduled dosing. Therefore, the concept of a “missed dose” does not generally apply. If you are using it on a scheduled basis under medical direction, take the missed dose as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.

Overdose

Acetaminophen overdose can be fatal and is a medical emergency. Early symptoms of overdose may include nausea, vomiting, loss of appetite, sweating, and general malaise. These may subside, leading to a false sense of improvement. Within 24-72 hours, clinical evidence of liver toxicity may appear (pain in the upper right quadrant, jaundice, coagulopathy). In case of known or suspected overdose, seek immediate medical attention or contact a Poison Control Center (1-800-222-1222) immediately, even if the patient feels well. Prompt administration of the antidote, N-acetylcysteine (NAC), is critical to prevent irreversible liver damage and can be highly effective if given within 8-10 hours of ingestion.

Storage

  • Store at room temperature, 20°-25°C (68°-77°F).
  • Protect from excessive moisture and heat. Do not store in bathrooms.
  • Keep the bottle tightly closed and out of reach of children and pets to prevent accidental ingestion.
  • Safely discard any expired or unused medication.
