Dosage and direction
The recommended dosage is the inhalation of the contents of one Tiova capsule (18 mcg tiotropium) once daily, at the same time each day, using the Tiova Inhaler device. The capsule must only be used with the designated inhaler and must not be swallowed. Instructions for use:
- Open the dust cap and mouthpiece.
- Place the capsule in the chamber; close the mouthpiece until a click is heard.
- Hold the inhaler upright and press the piercing button fully once.
- Breathe out fully, place mouthpiece between lips, and inhale deeply and steadily.
- Hold breath for 10 seconds, then exhale slowly away from the device.
- Repeat inhalation to ensure full dose delivery if needed.
- Open and discard the empty capsule; close the device.
Precautions
- Not for use in the treatment of acute bronchospasm; always have a rescue inhaler (e.g., SABA) available.
- Use with caution in patients with narrow-angle glaucoma, prostatic hyperplasia, or bladder neck obstruction due to anticholinergic effects.
- Monitor for signs of paradoxical bronchospasm; discontinue immediately if it occurs.
- Avoid getting powder in the eyes, as it may cause blurring of vision or pupil dilation.

Overdose
Overdose may lead to exaggerated anticholinergic effects such as dry mouth, visual disturbances, tachycardia, urinary retention, and constipation. Severe overdose could result in cardiac arrhythmias or central nervous system effects. Treatment is symptomatic and supportive; there is no specific antidote. Dialysis is not effective due to high protein binding and extensive tissue distribution.
Storage
Store below 25°C in a dry place. Protect from moisture and direct light. Keep capsules in the blister until immediately before use. Do not store the inhaler device with a capsule inside. Keep out of reach of children and pets. Do not use after the expiration date.
