Terramycin: Broad-Spectrum Antibiotic Ointment for Effective Infection Control

Terramycin (oxytetracycline with polymyxin B sulfate) is a time-tested, prescription-only ophthalmic and topical antibiotic ointment. Formulated for both ocular and dermatological use, it combats a wide array of susceptible gram-positive and gram-negative bacteria. Its dual-antibiotic mechanism ensures comprehensive coverage, making it a trusted choice for clinicians managing bacterial infections. This product is indicated for use when the causative organisms are susceptible to its components.

Features

  • Contains two potent antibiotics: Oxytetracycline HCl (5 mg/g) and Polymyxin B Sulfate (10,000 units/g).
  • Available in a sterile, preservative-free ointment base for sensitive application sites.
  • Provides a broad spectrum of antibacterial activity against common pathogens.
  • Ointment formulation ensures prolonged contact time with the affected area for sustained therapeutic effect.
  • Packaged in a tamper-evident, sealed tube to guarantee sterility and product integrity.

Benefits

  • Dual-Action Efficacy: The synergistic combination targets bacterial protein synthesis and disrupts cell membrane integrity, reducing the risk of treatment failure.
  • Rapid Infection Resolution: Effectively reduces bacterial load, alleviating symptoms like redness, swelling, discharge, and discomfort promptly.
  • Prevents Secondary Infections: Creates a protective barrier on application sites, guarding against opportunistic pathogens during the healing process.
  • Minimal Systemic Absorption: When used topically as directed, systemic exposure is negligible, focusing treatment on the localized infection.
  • Proven Clinical Reliability: Decades of clinical use and evidence support its safety and effectiveness for approved indications.

Common use

Terramycin Ointment is primarily prescribed for the treatment of superficial ocular infections caused by susceptible microorganisms, such as bacterial conjunctivitis, blepharitis, and blepharoconjunctivitis. It is also used for the prophylaxis of ocular infections following minor trauma, foreign body removal, or burns. In dermatological practice, it is applied topically to treat susceptible primary or secondary skin infections, including impetigo, folliculitis, and infected dermatoses. Its use is always contingent upon the suspected or confirmed susceptibility of the causative organism.

Dosage and direction

For ocular infections: Apply a approximately half-inch (1.25 cm) ribbon of ointment into the affected eye(s) every 2 to 4 hours, depending on the severity of the infection, or as directed by a physician. The frequency may be reduced as the infection improves. For topical skin infections: Apply a thin layer to the affected area 2 to 4 times daily, or as directed by a physician. Administration Instructions: Wash hands thoroughly before and after application. For eyes, gently pull down the lower eyelid to create a pouch, apply the ointment, and close the eye for 1-2 minutes. Roll the eyeball to distribute the medication. Avoid allowing the tube tip to contact any surface, including the eye or skin, to prevent contamination. Continue use for the full duration prescribed, even if symptoms appear to have resolved.

Precautions

  • This is a prescription medication. Use only under the supervision of a qualified healthcare professional.
  • Prolonged use may result in overgrowth of non-susceptible organisms, including fungi. If superinfection occurs, discontinue use and institute appropriate therapy.
  • Patients should be advised not to wear contact lenses while exhibiting signs and symptoms of an ocular infection or while using this ointment.
  • Ointments may cause temporary blurring of vision immediately after application. Exercise caution if driving or operating machinery directly after use.
  • For external use only. Do not ingest. Not for injection.
  • Use in pregnancy and lactation should only be undertaken if the potential benefit justifies the potential risk to the fetus or infant, as safety has not been definitively established.

Contraindications

Terramycin is contraindicated in patients with a known history of hypersensitivity to oxytetracycline, polymyxin B, or any other component of the formulation. Its use is also contraindicated in the treatment of mycobacterial infections, viral diseases (e.g., herpes simplex, vaccinia, varicella), and fungal diseases of the eye or skin. It should not be used in individuals with a history of severe renal impairment due to potential, though unlikely with topical use, systemic effects.

Possible side effect

While generally well-tolerated, the following side effects may occur:

  • Ocular: Temporary blurred vision, burning, stinging, itching, or irritation upon application; superficial punctate keratitis; redness; conjunctival erythema.
  • Dermatological: Localized skin irritation, itching, redness, rash, or contact dermatitis at the application site.
  • Hypersensitivity Reactions: Allergic reactions, though rare, may include pruritus, angioedema, skin eruption, and anaphylaxis. Discontinue use immediately if signs of hypersensitivity appear.
  • As with other antibiotic preparations, prolonged use may result in overgrowth of non-susceptible organisms.

Drug interaction

Formal topical interaction studies have not been conducted. However, the potential for interactions exists. Concomitant use with other topical ocular or skin products may alter the absorption and efficacy of each product. It is recommended to allow at least 5-10 minutes between applications of different topical medications. Systemically administered antibiotics may theoretically alter the gut flora, potentially reducing the efficacy of oral contraceptives; advise patients to use an alternative non-hormonal method of contraception. Always inform your physician of all other medications you are taking.

Missed dose

Apply the missed dose as soon as you remember. However, if it is almost time for the next scheduled dose, skip the missed dose and resume your regular dosing schedule. Do not apply a double dose to make up for a missed one.

Overdose

Acute overdose from topical application is unlikely due to minimal systemic absorption. Excessive application to the eye may result in increased irritation, conjunctival chemosis, or blurred vision. Treatment should be symptomatic and supportive. If accidentally ingested, seek immediate medical attention or contact a Poison Control Center. Symptoms of oral ingestion may include nausea, vomiting, and diarrhea.

Storage

Store at controlled room temperature, 20°C to 25°C (68°F to 77°F). Do not freeze. Keep the tube tightly closed when not in use. Store in the original container to protect from light and moisture. Keep out of reach of children and pets. Discard any unused medication after the course of therapy is completed. Do not flush down the toilet or pour down a drain.
