Common use

Protonix is primarily indicated for the short-term treatment (up to 8 weeks) of erosive esophagitis associated with GERD. It is also used for maintenance therapy of healed erosive esophagitis and for the long-term treatment of pathological hypersecretory conditions, including Zollinger-Ellison syndrome. Additionally, it may be prescribed as part of combination therapy for Helicobacter pylori eradication when used with antibiotics.

Dosage and direction

The recommended adult dosage for erosive esophagitis is 40 mg once daily for up to 8 weeks. For maintenance of healed erosive esophagitis, 40 mg once daily is typical. For pathological hypersecretory conditions, the dosage may be titrated based on individual patient response, with some patients requiring 80 mg twice daily. Tablets should be swallowed whole with water before a meal, preferably in the morning. Do not crush, chew, or split tablets as this may destroy the delayed-release properties.

Precautions

Patients should inform their healthcare provider of any liver disease or low magnesium levels before starting Protonix. Long-term use (more than one year) may increase the risk of fractures of the hip, wrist, or spine. There is also a potential risk of vitamin B12 deficiency with prolonged therapy. Patients should be monitored for signs of lupus erythematosus or cutaneous lupus erythematosus, as PPIs have been associated with new onset or exacerbation of existing conditions. Use with caution in patients with osteoporosis or those taking concomitant medications that affect bone metabolism.

Contraindications

Protonix is contraindicated in patients with known hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation. It should not be used in patients taking rilpivirine-containing products due to potential for reduced antiviral efficacy. Concomitant use with atazanavir is not recommended as pantoprazole may significantly reduce atazanavir plasma concentrations.

Possible side effect

Common side effects may include headache, diarrhea, nausea, abdominal pain, vomiting, flatulence, dizziness, and arthralgia. Serious side effects requiring medical attention may include severe diarrhea, kidney problems, low magnesium levels (manifesting as muscle spasms, irregular heartbeat, or seizures), and vitamin B12 deficiency with long-term use. Cutaneous and systemic lupus erythematosus have been reported with PPI use. Patients should contact their healthcare provider if they experience persistent watery diarrhea, abdominal cramping, or fever.

Drug interaction

Protonix may interact with drugs whose absorption is pH-dependent, including ketoconazole, iron salts, and digoxin. It may increase exposure to methotrexate. Concomitant use with warfarin may require increased monitoring of INR. Pantoprazole may reduce the antiviral activity of atazanavir and rilpivirine. It may also interact with tacrolimus and saquinavir. Patients should inform their healthcare provider of all medications, including prescription drugs, over-the-counter medications, and herbal supplements.

Missed dose

If a dose is missed, it should be taken as soon as remembered unless it is almost time for the next scheduled dose. In that case, skip the missed dose and resume the regular dosing schedule. Do not double the dose to make up for a missed one.

Overdose

Experience with pantoprazole overdose is limited. Doses up to 240 mg daily have been administered without reported adverse effects. In case of suspected overdose, symptomatic and supportive care is recommended. Dialysis is not likely to be effective due to high protein binding. Contact a poison control center or seek immediate medical attention if overdose is suspected.

Storage

Store at room temperature (20-25°C or 68-77°F), with excursions permitted between 15-30°C (59-86°F). Keep in the original container with the lid tightly closed to protect from moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
