Nurofen is a well-established, over-the-counter medication with the active ingredient ibuprofen, a nonsteroidal anti-inflammatory drug (NSAID). It is a first-line therapeutic choice for the effective management of mild to moderate pain, the reduction of fever (antipyresis), and the alleviation of inflammation. Its mechanism of action, inhibiting the production of prostaglandins, provides a targeted approach to treating the root cause of pain and swelling, not just masking the sensation. This profile offers a comprehensive, expert-level overview of Nurofen for informed use.
Features
- Active Ingredient: Ibuprofen (typically 200mg per tablet in standard OTC formulations).
- Drug Class: Nonsteroidal Anti-Inflammatory Drug (NSAID).
- Formulations: Available as film-coated tablets, caplets, liquid capsules, suspension/oral liquid for children, and topical gels.
- Mechanism of Action: Potent inhibitor of cyclooxygenase (COX) enzymes, specifically COX-1 and COX-2, which are responsible for prostaglandin synthesis.
- Onset of Action: Analgesic effects are typically felt within 30 minutes; anti-inflammatory effects may take up to 7 days of regular dosing for full efficacy in chronic conditions.
- Duration of Action: Provides relief for approximately 4 to 6 hours per dose.
Benefits
- Provides effective relief from a wide range of pain types, including headache, dental pain, backache, and muscular aches.
- Directly reduces inflammation at the site of injury or condition, addressing the underlying cause of pain and swelling.
- Effectively reduces elevated body temperature associated with colds, flu, and other infections.
- Offers a non-opioid analgesic option with a well-understood safety profile when used as directed.
- Available in multiple formulations to suit different patient needs and preferences (e.g., faster absorption with liquid capsules).
- Can be used for symptomatic relief of pain and inflammation in conditions like osteoarthritis and rheumatoid arthritis.
Common use
Nurofen is commonly used for the short-term symptomatic relief of:
- Headaches, including tension-type headaches and migraines.
- Menstrual cramps (dysmenorrhea).
- Toothache and pain following dental procedures.
- Backache, muscle aches, and pain associated with sprains and strains.
- The pain and inflammation of arthritis.
- Fever (pyrexia) associated with the common cold, influenza, and other viral or bacterial infections.
Dosage and direction
For Adults and Children over 12 years:
- The usual dose is 200mg to 400mg (1 to 2 tablets) up to three times a day as required.
- Doses should be taken with a minimum interval of 4 hours between administrations.
- Do not exceed 1200mg (6 tablets) in any 24-hour period.
- Take the lowest effective dose for the shortest duration necessary to control symptoms.
- Should be taken with food or a drink of milk to minimize the potential for gastrointestinal discomfort.
For Children:
- Use only formulations specifically designed for children (e.g., Nurofen for Children suspension).
- Dosage is based on the child’s body weight. Always follow the package instructions or a doctor’s/pharmacist’s advice precisely.
- Generally not recommended for children under 6 months of age or under a certain weight without medical consultation.
Precautions
- Gastrointestinal Risk: NSAIDs, including ibuprofen, are associated with an increased risk of serious gastrointestinal adverse events including bleeding, ulceration, and perforation, which can be fatal. This risk is higher in the elderly, those with a history of GI disease, and with prolonged use.
- Cardiovascular Risk: Long-term use of NSAIDs may increase the risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke, which can be fatal. This risk may increase with duration of use and in patients with pre-existing cardiovascular disease.
- Renal Function: Ibuprofen can cause renal toxicity. Use with caution in patients with impaired renal function, heart failure, hypovolemia, or taking diuretics.
- Asthma: Can exacerbate asthma, particularly in patients sensitive to aspirin and other NSAIDs.
- Pregnancy and Lactation: Do not use during the third trimester of pregnancy. Use during the first and second trimesters and while breastfeeding should only be under the direct advice of a physician.
- Elderly: Elderly patients are at a greater risk for serious GI, cardiovascular, and renal adverse events.
Contraindications
Nurofen is contraindicated in patients with:
- Known hypersensitivity (allergy) to ibuprofen, aspirin, or any other NSAID.
- A history of asthma, rhinitis, or urticaria induced by NSAIDs.
- Active peptic ulceration or a history of recurrent ulceration/bleeding.
- Severe heart failure, severe hepatic failure, or severe renal failure.
- Third trimester of pregnancy.
- In the context of other specific medical conditions as determined by a healthcare professional.
Possible side effect
Like all medicines, Nurofen can cause side effects, although not everybody gets them. Common (may affect up to 1 in 10 people):
- Gastrointestinal: Dyspepsia, heartburn, nausea, abdominal pain.
- Central Nervous System: Headache, dizziness.
- Other: Rash.
Uncommon (may affect up to 1 in 100 people):
- Gastrointestinal: Diarrhea, flatulence, constipation, vomiting.
- Dermatological: Urticaria (hives), pruritus (itching).
Rare/Serious (seek immediate medical attention if experienced):
- Signs of GI bleeding: Hematemesis (vomiting blood), melena (black, tarry stools), severe abdominal pain.
- Signs of allergic reaction: Swelling of the face, lips, tongue, or throat (angioedema), difficulty breathing, severe skin reactions (e.g., Stevens-Johnson syndrome).
- Signs of cardiovascular events: Chest pain, shortness of breath, weakness in one part of the body, slurred speech.
- Signs of renal problems: Passing less urine than usual, swelling of ankles/feet (edema).
- Signs of hepatic dysfunction: Jaundice (yellowing of skin/eyes), dark urine.
Drug interaction
Ibuprofen can interact with many other medicines. Inform your doctor or pharmacist if you are taking:
- Other NSAIDs: Including aspirin and COX-2 inhibitors (increased risk of side effects).
- Anticoagulants: Like warfarin, dabigatran (increased risk of bleeding).
- Antihypertensives: Such as ACE inhibitors (e.g., lisinopril), angiotensin-II receptor antagonists (e.g., losartan), and beta-blockers (e.g., atenolol); ibuprofen may reduce their effectiveness.
- Diuretics: (e.g., furosemide, hydrochlorothiazide); reduced diuretic efficacy and risk of renal impairment.
- Corticosteroids: (e.g., prednisolone); significantly increased risk of GI ulceration.
- SSRIs: (e.g., citalopram, fluoxetine); increased risk of gastrointestinal bleeding.
- Methotrexate: Can decrease methotrexate clearance, increasing its toxicity.
Missed dose
Nurofen is taken on an “as-needed” basis for pain relief. It is not a scheduled medication. Therefore, if you miss a dose and are still in pain, you may take it when you remember, provided it has been at least 4 hours since your last dose. Do not double the dose to make up for a missed one.
Overdose
- Symptoms: Nausea, vomiting, epigastric pain, lethargy, drowsiness. More severe overdose can lead to metabolic acidosis, gastrointestinal bleeding, hypotension, acute renal failure, respiratory depression, and coma.
- Action: In case of overdose, seek immediate medical attention or contact a Poison Control Center. Do not induce vomiting unless instructed by a medical professional. Management is supportive and may include gastric lavage, activated charcoal, and monitoring of vital organ function.
Storage
- Store in the original container below 25°C (77°F).
- Keep out of sight and reach of children.
- Do not use after the expiry date printed on the pack.
- For liquid suspensions, some formulations may require shaking before use. Refer to the specific product instructions.
