FML Forte (fluorometholone ophthalmic suspension) 0.1% is a high-potency topical corticosteroid formulated for the management of severe steroid-responsive inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe. It is indicated when a robust anti-inflammatory and immunosuppressive effect is required to control significant ocular inflammation that is unresponsive to weaker steroid preparations. Its enhanced formulation delivers a concentrated therapeutic effect, targeting the underlying inflammatory cascade to reduce edema, fibrin deposition, capillary dilation, and phagocytic migration. FML Forte is a critical tool in the ophthalmologist’s armamentarium for preserving visual function and ocular integrity in the face of aggressive inflammatory disease.

Common use
FML Forte is prescribed for the short-term treatment of severe steroid-responsive inflammatory conditions of the eye. Its common uses include, but are not limited to, allergic conjunctivitis, acne rosacea, superficial punctate keratitis, herpes zoster keratitis, iritis, cyclitis, selected infective conjunctivitis (when the inherent hazard of steroid use is accepted to obtain a diminution in edema and inflammation), and the prevention of corneal neovascularization post-chemical, thermal, or radiation burns. It is also used to manage post-operative inflammation following surgical procedures such as cataract extraction and refractive surgery.
Dosage and direction
Shake the bottle well before each use. The dosage of FML Forte must be individualized based on the severity of the inflammatory condition.
- Initial Treatment: Instill 1 to 2 drops into the conjunctival sac of the affected eye(s) every 1 to 2 hours during the day and every 2 hours during the night until a satisfactory clinical response is obtained.
- Maintenance Dose: After a favorable response is observed, reduce dosage to 1 drop 2 to 4 times daily. The frequency of administration should be decreased gradually, consistent with established clinical principles for corticosteroid tapering.
- Treatment Duration: Treatment should not be discontinued abruptly. The duration of treatment will vary but typically should not exceed 2 weeks for most conditions. Prolonged use requires careful slit-lamp monitoring and tonometry.
- To prevent contamination of the dropper tip and solution, care should be taken not to touch the eyelids or surrounding areas with the dropper tip of the bottle. Keep the bottle tightly closed when not in use.
Precautions
- Prolonged Use: May increase the risk of elevated intraocular pressure (IOP), which may result in glaucoma, optic nerve damage, and defects in the visual fields. It may also increase the susceptibility to secondary ocular infections from fungi or viruses.
- Steroid Response: Monitor IOP in all patients, even though a steroid response is less common with fluorometholone than with dexamethasone. Patients should be re-evaluated if the clinical signs and symptoms do not improve after 2 days.
- Perforation: Use with extreme caution in patients with corneal thinning or those with a history of corneal ulceration, as perforation has been known to occur with the use of topical corticosteroids.
- Cataracts: Posterior subcapsular cataract formation has been reported with prolonged use of corticosteroids.
- Contact Lenses: Patients should be advised not to wear contact lenses during the course of treatment with FML Forte.
- Bacterial Infection: If an infection does not respond promptly to appropriate therapy, discontinue use until the infection has been adequately controlled.
Contraindications
FML Forte is contraindicated in:
- Most viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella.
- Mycobacterial infection of the eye.
- Fungal diseases of the ocular structures.
- Hypersensitivity to any component of this medication (fluorometholone, benzalkonium chloride, or other ingredients).
- Purulent, untreated ocular infections.
Possible side effect
The following adverse reactions may occur, listed by system:
- Ocular: Elevated intraocular pressure (IOP) with possible development of glaucoma and infrequent optic nerve damage; posterior subcapsular cataract formation; secondary ocular infection from fungi or viruses liberated from ocular tissues; corneal perforation in thin corneas; transient stinging or burning upon instillation; blurred vision; ocular discomfort/irritation; foreign body sensation; conjunctival hyperemia; punctate keratitis; mydriasis; ptosis.
- Non-ocular: Systemic effects are rare with topical use but are possible, especially with prolonged use, and may include manifestations of hypercortisolism.
Drug interaction
- Although minimal systemic absorption occurs, potential interactions exist with other systemic corticosteroids a patient may be taking.
- Inform your physician of all medications you are using, including prescription, over-the-counter, vitamins, and herbal products.
Missed dose
If a dose is missed, instill it as soon as you remember. If it is almost time for the next dose, skip the missed dose and continue with your regular dosing schedule. Do not instill a double dose to make up for a missed one.
Overdose
Topical overdose of FML Forte is unlikely to cause acute, life-threatening symptoms. The bottle contains a relatively small amount of medication. If accidentally ingested, seek medical attention or contact a Poison Control Center immediately. Ocular overdose may be flushed from the eye(s) with warm tap water.
Storage
- Store at controlled room temperature 15°-30°C (59°-86°F).
- Protect from freezing and direct light.
- Keep the bottle tightly closed when not in use.
- Discard the bottle and any remaining medication 28 days after first opening to prevent contamination and potential infection. Write the discard date on the bottle label when you open it.
