Cardura (doxazosin mesylate) is an alpha-1 adrenergic blocker medication prescribed for the management of hypertension (high blood pressure) and the treatment of symptomatic benign prostatic hyperplasia (BPH). It functions by relaxing blood vessels and muscles in the prostate and bladder neck, facilitating smoother blood flow and improved urinary function. This dual-action mechanism makes it a versatile option in a clinician’s therapeutic arsenal for addressing cardiovascular and urological conditions.

Common use
Cardura is primarily indicated for two conditions. It is effective in treating both the obstructive symptoms (e.g., straining, weak stream, incomplete emptying) and irritative symptoms (e.g., nocturia, urgency, frequency) associated with BPH. Treatment aims to improve urinary flow and reduce complications like acute urinary retention.
Dosage and direction
The dosage of Cardura must be individualized based on the patient’s therapeutic response and tolerability.
For Hypertension:
- The initial dose is 1 mg administered once daily.
- The dose may be increased to 2 mg after 1-2 weeks, and thereafter to 4 mg and 8 mg if necessary, based on blood pressure response.
- The maximum recommended dose is 16 mg daily, though most patients experience an adequate response at 4-8 mg daily.
- Dose titration should occur at intervals of 1-2 weeks.
For Benign Prostatic Hyperplasia:
- The initial dose is 1 mg administered once daily, preferably at bedtime.
- The dose may be increased to 2 mg, 4 mg, and 8 mg once daily, based on the patient’s symptomatic response and tolerability.
- The recommended titration interval is 1-2 weeks.
- The maximum recommended dose for BPH is 8 mg once daily.
The tablet should be swallowed whole with a glass of water, with or without food. Consistency in the time of administration is advised.
Precautions
Patients should be advised about the potential for orthostatic hypotension (a sudden drop in blood pressure upon standing), especially with the first dose, during dosage titration, and after interruption of therapy. This can manifest as dizziness, lightheadedness, or syncope (fainting). To minimize risk, the initial dose and the first dose after any increase should be taken at bedtime. Patients should avoid driving or operating hazardous machinery for 12-24 hours after the first dose or a dose increase. Caution is advised when commencing therapy in patients with known severe hepatic impairment. Regular monitoring of blood pressure and, in BPH patients, prostate-specific antigen (PSA) levels is recommended. Patients should inform their surgeon of their Cardura use prior to undergoing cataract surgery due to the risk of Intraoperative Floppy Iris Syndrome (IFIS).
Contraindications
Cardura is contraindicated in patients with a known hypersensitivity to doxazosin, other quinazolines (e.g., prazosin, terazosin), or any component of the formulation. Its use is also contraindicated in patients with a history of orthostatic hypotension and as a treatment for benign prostatic hyperplasia in women and children.
Possible side effect
Like all medications, Cardura can cause side effects, although not everybody gets them. Common side effects are often related to its vasodilatory effects and may include:
- Dizziness (15.6%)
- Fatigue (8.2%)
- Headache (7.8%)
- Somnolence (drowsiness) (5.4%)
- Nausea (3.9%)
- Orthostatic hypotension (dizziness when standing, 1.3%)
- Edema (swelling, 1.7%)
- Rhinitis (nasal congestion, 1.9%)
- Palpitations (1.3%)
Less common but more serious side effects can include syncope (fainting), priapism (a prolonged and painful erection), and severe hypotension. Patients experiencing any severe or persistent side effects should contact their physician immediately.
Drug interaction
Concomitant use of Cardura with other antihypertensive agents or phosphodiesterase-5 inhibitors (e.g., sildenafil, tadalafil) used for erectile dysfunction or pulmonary hypertension may potentiate the blood-pressure-lowering effects and increase the risk of hypotension. Caution is warranted when administering Cardura with strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, clarithromycin) as they may increase doxazosin plasma concentrations. Use with other alpha-adrenergic blocking agents is not recommended. Additive effects on blood pressure reduction may also occur when used with alcohol, benzodiazepines, or other CNS depressants.
Missed dose
If a dose is missed, it should be taken as soon as remembered on the same day. If it is not remembered until the next day, the patient should skip the missed dose and take only the next regularly scheduled dose. Do not double the dose to make up for a missed one.
Overdose
In the event of an overdose, the primary expected manifestation is severe hypotension, which can lead to shock, syncope, and cardiovascular collapse. Other symptoms may include profound dizziness, drowsiness, and blurred vision. In cases of suspected overdose, supportive care is paramount. The patient should be placed in a supine position with their legs elevated to help restore blood pressure and facilitate blood flow to vital organs. Gastric lavage or activated charcoal may be considered if ingestion was recent. Vital signs and renal function should be monitored closely. IV fluids and vasopressors may be necessary to manage hypotension. As doxazosin is highly protein-bound, dialysis is not likely to be beneficial.
Storage
Store Cardura tablets at room temperature, between 20°C to 25°C (68°F to 77°F), in a dry place. Excursions are permitted between 15°C and 30°C (59°F and 86°F). Keep the medication in its original container, tightly closed, and out of reach of children and pets. Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed.
