Dosage and direction

The recommended dosage for narcolepsy or obstructive sleep apnea is 200mg taken orally once daily in the morning. For shift work sleep disorder, 200mg should be taken approximately one hour before the start of the work shift. Dosage may be adjusted based on efficacy and tolerability, within a range of 100–400mg daily. Tablets should be swallowed whole with water; they may be taken with or without food. Avoid afternoon dosing to prevent insomnia.

Precautions

Patients should be monitored for the emergence of new psychiatric symptoms including anxiety, agitation, or hallucinations. Cardiovascular status should be evaluated in those with pre-existing hypertension, arrhythmias, or structural heart disease. Use with caution in patients with a history of drug abuse, hepatic impairment, or renal dysfunction. Modaheal may impair judgment or motor skills; advise against driving or operating machinery until response is known. Pregnancy and lactation are contraindications unless potential benefit justifies risk.

Contraindications

Hypersensitivity to modafinil or armodafinil or any component of the formulation. History of left ventricular hypertrophy or mitral valve prolapse syndrome associated with CNS stimulants. Patients with symptomatic cardiovascular disease, advanced atherosclerosis, or uncontrolled hypertension. Concomitant use with monoamine oxidase inhibitors (MAOIs), or within 14 days of discontinuing MAOI therapy.

Possible side effect

Common (≥1%): headache, nausea, nervousness, insomnia, dizziness, diarrhea.
Less common: dry mouth, anorexia, anxiety, palpitations, elevated blood pressure, blurred vision.
Rare but serious: Stevens-Johnson syndrome, angioedema, multiorgan hypersensitivity, psychiatric symptoms (e.g., aggression, depression, psychosis), symptomatic hypertension.

Drug interaction

Modaheal is a moderate inducer of CYP3A4 and may reduce the efficacy of CYP3A4 substrates including hormonal contraceptives, cyclosporine, midazolam, and certain statins. Concurrent use with MAOIs is contraindicated. May potentiate effects of sympathomimetics. Can increase metabolism of warfarin, phenytoin, and diazepam. Alcohol may enhance cognitive and motor impairment.

Missed dose

If a dose is missed, it should be taken as soon as remembered unless it is close to the next scheduled dose. Doubling the dose is not recommended. For shift work disorder, if missed before the shift, take as soon as possible unless the shift is nearly over.

Overdose

Symptoms may include insomnia, agitation, anxiety, confusion, tachycardia, hypertension, and gastrointestinal distress. There is no specific antidote; management is supportive and symptomatic. Hemodialysis is not expected to enhance elimination. Contact a poison control center or emergency department immediately.

Storage

Store at room temperature (15–30°C) in a dry place, protected from light and moisture. Keep in the original blister packaging. Do not use after the expiration date printed on the packaging. Keep out of reach of children and pets.
